For more than two decades, Shawntá Crayton has been dedicated to providing compassionate, patient-centered care during life's most vulnerable moments. As a dual-certified Family Nurse Practitioner (FNP-C) and Psychiatric Mental Health Nurse Practitioner (PMHNP-C), she provides evidence-based, whole-person care for patients ages 12 years and up.

Her passion for healthcare has been shaped by personal experiences, patient advocacy, and witnessing gaps in care. Those experiences, along with the meaningful relationships she has built with patients throughout her career, inspired her to found Evolve Health Clinic—a practice built on compassion, connection, education, and whole-person healing.

Shawntá believes every patient deserves to feel respected, supported, and actively involved in their healthcare decisions. She provides personalized care for physical and mental health needs, including preventive care, acute illness, weight management, and mental health services. Her goal is to empower every patient with the knowledge, support, and individualized care they need to improve their health, build resilience, thrive, and become the best version of themselves—from the inside out.
